Position: Practice Nurse
Location: General Practice (Moderate-Sized Practice)
Reports To: GP Principal / Practice Manager
Contract: Permanent (Full-Time or Part-Time Considered)
Overview
We are seeking a motivated and patient-focused Practice Nurse to join our friendly and progressive general practice team. The successful candidate will play a key role in delivering high-quality nursing care within a primary care setting, with a particular emphasis on Chronic Disease Management (CDM), preventative healthcare, health promotion, and patient education.
Previous experience in general practice nursing is desirable but not essential. Applications are welcomed from registered nurses who are interested in developing a career in primary care and chronic disease management, with support and training provided.
Key ResponsibilitiesChronic Disease Management (CDM)
- Deliver nursing care as part of the HSE Chronic Disease Management Programme.
- Conduct structured reviews for patients with chronic conditions including:
- Type 2 Diabetes
- Asthma
- COPD
- Cardiovascular Disease
- Record clinical observations and measurements including:
- Blood pressure
- Weight and BMI
- Pulse and oxygen saturation
- Foot assessments
- Smoking status
- Assist with patient education, self-management support, and lifestyle counselling.
- Maintain accurate clinical documentation and coding.
General Practice Nursing Duties
- Immunisations and vaccination programmes.
- Cervical screening (where trained).
- Wound care and dressings.
- Phlebotomy and specimen collection.
- ECG recording.
- Health promotion and preventative screening.
- Women's and men's health clinics.
- Infection prevention and control.
- Medication administration within scope of practice.
Patient Care
- Provide safe, evidence-based nursing care.
- Promote patient wellbeing through education and health promotion initiatives.
- Maintain patient confidentiality and comply with GDPR requirements.
- Work collaboratively with GPs, administrative staff, and other healthcare professionals.
Administrative and Clinical Governance
- Maintain accurate electronic patient records.
- Participate in clinical audits and quality improvement initiatives.
- Ensure compliance with Nursing and Midwifery Board of Ireland (NMBI) standards and professional guidelines.
- Attend relevant training and continuing professional development activities.
Essential Criteria
- Registered General Nurse with current registration on the NMBI Register.
- Excellent communication and interpersonal skills.
- Strong organisational and time-management abilities.
- Ability to work independently and as part of a multidisciplinary team.
- Commitment to patient-centred care and professional development.
- Proficient computer skills and willingness to work with electronic medical record systems.
Desirable Criteria
- Previous experience in a General Practice or Primary Care setting.
- Experience in Chronic Disease Management/OCF/PP.
- Phlebotomy experience.
- Cervical screening qualification.
- Vaccination and immunisation training.
- Experience using GP clinical software systems.(Socrates)

Terms and BenefitsSalary
- Starting rate: €28.00 per hour
- Higher rates may be considered based on experience and qualifications.
